General & Family Physician 's  Response
Hi, welcome to HCM, I am Dr. Das.
The report shows normal liquefaction time , normal sperm count ( >40 million as per WHO 1999). MOtility of the sperm is also within normal limit ( > 50 % as per WHO 1999).
But the abnormal forms are marginally high .
Others are within normal limit.
So, overall, the report is enough for fertility.
