General Surgeon 's  Response
Hi,

15 months is a long time to suffer from anal fissure. It is recurrent and troubles you a lot.
I would advise you the following in such a situation:
Consult a General Surgeon or a Proctologist for per-rectal examination if there is less or no pain; ano-proctoscopy.
Colonoscopy should be done in all cases which are not getting well.
Tests of stool and blood.
A course of an antibiotic and Metronidazole help a lot.
Take an additional probiotic.
Have a full diet high in fiber contents.
Continue Sitz bath.
If all these additional measure do not help then it is best to undergo Lord's Anal dilatation under General Anesthesia. Or lateral sphincterotomy.
Hope I have answered your query. Let me know if I can assist you further.

Regards,
Dr. T Chandrakant,
General Surgeon
