Your sperm count is adequate. The sperm motility is good. Number of active sperms is good. Motility after 1st and 2nd hour is good.
The only issue is high number of White Blood Cells suggesting that there is infection in Male Reproductive Tract, may be in Prostate (Prostatitis) or Epididymis (Epididymitis).
You need to take a good antibiotic for around 3 months and then get the semen analysis done again. If normal you may stop the antibiotic.
