Hello,
Good day. TSH of 21 is abnormal. I am keen to know her Free T4 report too. You should also do an Anti TPO blood test. Does she have thyroid enlargement.
If she has no heart problems, you can start with Thyrox 75 mcg. It is safe. But if there is any history of heart problem, better to start with 50 mcg first and slowly after 15 days, increase to 75 mcg. It is a safe drug.
To take in empty stomach before food. Store Thyrox in a cool and dry place away from sun light and heat. Never store in Kitchen because the temperature there will be higher.
Recheck TSH after 6 weeks of starting the 75 mcg dose
