Is Adenoma Really Related To Diabetes?

Hello Doctor, My gandfather is suffering from a pituitary condition (edenoma). I am providing some history of his condition. Please suggest what to do. 1)     Age : 74 yrs .Diabetic for past 35 years. Have High Blood pressure. No other severe conditions. 2)     Hernia operation was done in year 2006 which failed. We have realized that Patient will power is not very strong and healing becomes challenge. 3)     Pitiutary Edenoma identified in 2013. At that time this was identified when some tests proved that he had Low Sodium and diabetes was very high. He was on oral tablets and then changed to insulin injection after all these identification. Vision tests did not show a remarkable effect on optic nerve. Doctors in Medanata wanted to do surgery. Patient did not agree. 4)     This year again his blood sugar levels are nearly 350 post meals even after insulin dosage. Neurosurgeon has recommended a surgery but we do not want to go ahead with his condition. Vision test is yet to be done. 5)     Hence request you to suggest : a)     Till what time or size of edenoma we can avoid the surgery situation? b)     Is Edenoma really related to diabetes or these are separate conditions? c)     Is Sodium fluctuation related to Edenoma?

As per my clinical experience, the following are answers as per your query requirements. The pituitary tumors can be classified as hormone secreting and non-hormone secreting. Pituitary adenoamas are usually benign conditions which can affect the body systems by way of hormone release or size impacting the surrounding brain structures.

The most common is pitutary adenoma which is known as Prolactinoma, where the high levels of prolactin hormone is associated with:
- Chronic headache
- Vision changes (if size above 1 cm compressing optic chiasm) in form of bitemporal hemianopsia
- Breast enlargement
- Infertility

The diabetes in concern with pitutary tumor is most likely related to ACTH secreting pitutary tumor, which is also responsible for the sodium fluctuations. If the size is considerable above 10 mm, symptoms are definitely in concern with the tumor, Transsphenoidal surgery of pituitary tumor is advisable after discussing the pros and cons of the surgery against medical management aspects.
