Hypothyroid On Levothyroxine. Suspected PCOS When Acanthosis Nigricans Was Seen On Neck. What Could This Be?

I'm a hypothyroid patient and have been on levothyroxine treatment for 1 month.My recent lab results show a normal TSH level and triiodothyronine and thyroxine are also normal.My doctor suspected PCOS when he saw acanthosis nigricans on my neck,an ultrasound of my ovaries came back normal.My recent periods were 45 days apart and my creatinine level,urea level are both zero in SI units.My FSH=5.88mIU/ml.LH=2.95mIU/ml and testosterone is 0.47ng/ml.What do you think?

Hello and welcome to HCM,

The levels of FSH, LH and testosterone are all within normal limits.
So, a diagnosis of PCOS is ruled out.
PCOS is a condition characterised by high LH and high testosterone levels.
Since, the ultrasound of the ovaries is normal, PCOS is ruled out.
Moreover PCOS is not associated with any skin lesion.
Acanthosis nigricans is a skin condition associated with underlying malignancy.
If your doctor is suspecting PCOS, a thorough clinical examination and investigations of all organ systems is needed.
For the confirmation of acanthosis nigricans, a biopsy of the lesion is also required.
