How To Treat PCOS?

I am 23 and due to lack of insurance have not officially been diagnosed with PCOS. I have gotten an exam and it was my doctor's opinion that I have PCOS but without insurance the full slew of procedures couldn't be afforded so I was put on birth control. Sure enough, my symptoms dissipated and I lost 15 pounds (With a diet change and increased physical activity) as I am also obese. But my period has started and I'm experiencing horrible symptoms. I bleed through 3 sanitary napkins an hour, I've lost 5 pounds in one day, and am having cramps that feel like contractions again, releasing massive clots every time, and I am extremely dizzy and short of breathe. I live in a state that has extremely limited Planned Parenthood access due to funding cuts and full appointments. I do NOT have money to go to a regular OB/GYN. Is there ANYTHING I can do for my symptoms or is this a serious condition that I need to find a way to address?
