General & Family Physician 's  Response
Hi,

Hormonal changes around the pubertal and teenage period can cause heavy bleeding with clots. If these symptoms are severe, other causes of hormonal imbalance should be identified. Hypothyroidism, anemia and uterine abnormalities should be ruled out. Hormonal pills may be required for a few cycles to regulate your bleeding. Anti-spasmodics can be taken to reduce pain or cramps.

Take care. Hope I have answered your question. Let me know if I can assist you further.

Regards,
Dr. Saranya Ramadoss, General & Family Physician
