General & Family Physician 's  Response
Hi Mazedul Huq,
Thanks for posting in HCM.
I understand your concern.
Hepatits B is a viral infection affecting the liver. Since you are found to be positive for it, it is certain that it would have affected the liver tissues.
When you are on medication, it does not completely cure the disease, but helps to keep it under control. On and off, the viral load could increase or decrease depending on the body's immune response. During the increase of viral load, some amount of liver tissue destruction would take place, which is reflected by an increase in enzyme SGPT (ALT) levels.
Hence, you need not be worried much about its fluctuations as the increase is only about 2 times than that of normal.
In case it increases 5 fold or more than the normal values, you need to be evaluated by means of other investigations like Liver Function Tests (LFT) and ultrasound scan.
Kindly continue the medication and do necessary follow-ups as advised.
Hope the information provided would be helpful.
All the best.
