CRF patients tend to accumulate fluid. So when there is fluid overload, they show breathing difficulty due to pulmonary edema. He is also having high urea. This suggest further worsening of CRF. So in my opinion, he should increase the frequency of dialysis (to avoid fluid over load), thrice weekly. He will mostly improve with breathing difficulty after three times a week dialysis.
