General Surgeon 's  Response
Hi, read and understood your history of severe pain in the testicle immediately on straining at stool, feeling like hit in the testicle and now left testicle is swollen, painful and tender.
I would take this as a case of torsion testis with the classical history of instigation where you should have been attended, urgent color doppler done to see the reason and be operated for the reversal within 6 hours of the attack.
It is OK as it is now already late, yet I would advise in such a situation to urgently undergo ultrasonography and color doppler examination, which will guide me whether to explore the testis and do un-torsion or to remove it if it is already black and not reversible.
The chance of this being dye to infection is rare as you do not symptoms of fever, chills, urinary symptoms.
Rush up.

General Surgeon Dr. T Chandrakant's  Response
Hi.
Thanks for your query.
Read and understood that you have got swelling of the left testicle after straining at the bowel motion.
This is indicative of a testicular torsion and you need to rush to the ER.
You have to have urgent ultrasonography and color doppler and to get surgery done immediately to save the testicle if there is torsion.
