Hi,
With your menstrual history and the present symptoms, it appears more likely to be a hormonal imbalance. Moreover, urine pregnancy tests may not be of much help as your period of ovulation cannot be predicted. Serum beta-hCG titre estimation is much better. Also, a trans-vaginal sonogram should be taken to exclude other pelvic causes like polycystic ovaries and exclude/confirm pregnancy. If pregnancy has been excluded, you should be treated with progesterone supplements and further management planned. Hope this helps.
