Does A Rapidly Declining GFR Warrant Immediate Medical Attention?

What would cause a rapid decline in GFR? My GFR WAS 78, 5/1, GFR 56 7/31 with creatinin 1.05, GFR 39 on 9/22 with creatinin 1.42. My Dr has given me a referral to a nephrologist, but they said it may be a couple of weeks or more. I am a 66 year old female, I recently started taking metformin for high fasting glucose, but i have not been diagnosed with DM. I have high cholesterol and high triglycerides. Which I take Fenofribrate and livalo. I am also obeses. I am also a retired dialysis RN so I do not want to damage my kidneys by waiting for 2-3weeks. Is this an emergent situation?
