How Can Severe Anxiety Be Treated?

Do anxiety attacks make me feel almost like I am out of myself..ie. third person? Do all noises around me get heightened? My vision is at times affected.
I have to leave the area I am in. Putting on my sunglasses seem to help at times.
A retired pshyciatrist told me that I have floating anxiety attacks(?)
What is my best choice of how to handle this? I do not have insurance at this time as I cannot afford the deductible.
