Child Has Tiredness And Disorientation. No History Of Trauma. Further Evaluation?

8 yo WM at home complaining of being tired, lethargic per mom doesn't recognize Mom and not oriented place as well.
Pt. taken to local general ED where he was noted per Mom to be disoritentd unable to stand, remained lethargic, vs - nl, no hx of trauma, prior hx of similar presentations, - fever. No change recently in meds. Pt. transferred to local Children's hospital where despite being phoned my an MD from the transferring ED and having been sent via EMS, the pt. was not evaluated for 4 hrs while waiting. Over the ensuing 4 hrs. the pt became much more alert but still not quite himself. MD after evaluation sent pt home without a CT or MRI of the head and with an excuse to return to school after the weekend as well as being free to return to physical education. Your thoughts. As a pediatrician, concerned regarding lack of further eval. in view of - hx. YYYY@YYYY / Kim Boggs/ Knoxville, TN

This episode of tirednedd, loss of orientation needs further evaluation.
History should focus on seizures, ingestion of sedative medication or any other drugs.
Physical examination usually gives clue to the underlying abnormality.
Investigations like CT scan brain, EEG, blood for toxic screen will help to find out thr reason.
I suggest ypu to take the child to his Pediatrician for further evaluation.
