Hi,
I am Dr Mittal.
I have read your message.
I think I can help you.

There are 4 parts to your query.
First, the control of Blood pressure.
The new medicine has definitely brought down the blood pressure to a more acceptable range.
I suggest continuing the same medicine.
Besides, by some guidelines, beta blockers (metolar) are not suggested unless there is a related heart problem.
So definitely your new medicine is better and safe.

Second, is there a side effect?
However, there is a diuretic in the medicine.
So monitor your sodium monthly.
Low sodium is a definite side effect of the diuretic.
In fact, I usually prefer to give plain olmesartan (olmazest) first and then gradually step up to diuretic if needed.
I suggest the same to you, but you will need to monitor BP regularly if you make the change.
You may discuss this with your prescribing physician.

Third, yes the medicine can be taken in diabetes.
In fact metolar is known to increase the blood sugars by 2-5 points.
So the change is overall good for you.

Finally, isomnia or the problem with sleep.
Now here I need more details.
First, do you have problem initiating sleep or do you sleep normally and get up fast?
Second, is there a physical discomfort like pain or breathlessness or chest discomfort or palpitations?
Third, is there any ongoing tension?
Fourth, do you snore? What is your weight and height?
I suggest the following measures.
1. Avoid any work or TV or computer for at least half hour before sleeping.
2. Avoid alcohol or coffee prior to sleep.
3. Try sleeping by your side.
4. Make sure your mattress and pillow is cofortable.
5. Try meditation prior to sleeping.
6. Make a list of pending things before you sleep.
7.Get a sleep study.
8. Meet your practitioner for a prescription for mild sedatives to be taken on sos basis.
Let me know the requested details for further information.

I have tried to make it as simple as possible.
Feel free to contact us for more information.
Best of luck, Dr Mittal
