Are Shortness Of Breath, Diarrhea And Sleepiness Side Effects Of Metoprolol?

Harvey has blood pressure of 182 over 56. l. Two weeks ago he was taken by ambulance to the hospital for an elevated heart rate. The hospital did a cardioversion and released him. Prior to going to the hospital he took 25 mg. of Metoprolol but it didn t help him. He had been prescribed Metroprolol five year prior to be taken in the event of an elevated heart rate. He had only taken the drug once in five years prior. 2. Three days later he had the same problem and went to the hospital but when he arrived his heart rate was normal because he took 25 mg. of Metoprolol twice, a half hour apart and he was released from the hospital. 3. Two days later his heart and blood pressure was extremely high but he did not go back to the hospital. 4. The next day his pressure was 233 over 190 and his pulse was 133 so he was once again taken by ambulance to the hospital where they admitted him and after a urine test they found blood in his urine and an infection. They started him on Macrobid 100 mg. twice a day for 5 days. 5. Harvey was admitted to the hospital for a heart problem and although he had an urinary infection and he was unable to urinate and had to have a catheter the hospital was restricted from investigating the urinary problem therefore they only treated his heart problem, despite the fact that the bag that filled with urine was bright red with blood. 6. After four days the new medication that Harvey was receiving for his heart (amiodarone 800 mg a day & metoprolol 50 mg a day) stabilized his heart and he was released from the hospital. He immediately went to a urologist to investigate his urinary problem. During his time in the emergency department of the hospital his cardiologist thought his heart rate was elevated due to his urology problem. I think the other doctors felt that his heart problem was separate from him urology issue and although his cardiologist had suggested the urology problem may have caused his elevated heart problem they disagreed and she then prescribed amiodarone and metoprolol. Since Harvey started these new drugs his blood pressure is very low and his pulse is around 40 beats a minute. He is unwell; has vomited, has diarrhea, is lightheaded, has shortness of breath, sleeps during the day, has vivid dreams, and is feeling unwell. What do you think of his present pressure?

General & Family Physician 's  Response
Hi,

Presently, his main symptoms are related to low B,P. as a result of overdose of medicines. His medicines are to be reduced depending on his Blood pressure and pulse rate. We may first try to reduce Metoprolol to 25 mg daily and if indicated completely weaning from Metoprolol. Detailed cardiac work up should be done when he is stabilized. This work up may include Echocardiogram, TMT and also angiography if indicated.

Take care. Hope I have answered your question. Let me know if I can assist you further.

Regards,
Dr. Satnarain Bansal, General & Family Physician
