General & Family Physician 's  Response
Hi,

Waking up with mouth full of blood could be related to sore and swollen infected tonsils.

Another cause could be related to lung problems, as complication of high blood pressure and/or diabetes.

In such cases, his state should be further evaluated by an ENT specialist. If nothing, then, should :
- consult cardiologist
- run EKG, cardiac echo
- do chest X-ray

All the best to your friend!
