Question: i am a 36 yr old female with mutiple symptoms , Looking for ideas of what next route of treatment would be

12 months ago for the first time had 1 episode of Tachicardia 150bpm , treated in A n E and had outpatient investigations , x ray, 24 hr moniter , ultrasound of heart , bloods All Apparently normal
so was advised was likely a one off thing .....

8 weeks ago , i was again at A n E where i have now been 3 times alongside visits to my Gp
again i have
Tachicardia / high bp
i have had bloods done in hospital including thyroid all normal
xray ,
ekg 3 times , normal
24 hour moniter , normal ( picked up an extra beat apparently )
my gp has prescribed bisoprolol 2.5 mg to lower heart rate and BP to try and control for now
and im now going to see an Endocrinologist to rule out adrenal causes ( this is in the process )

i have blurry vision , ache in the back of my head ,tightness in chest , breathless with wheezing now and then , cramps in my left arm , recently started with indigestion / burning throat / nausea
i have been given Lansoprazole 15mg to try for the gastro issue , most of the above i would say ive only noticed since starting the bisoprolol

im just concerned that if the endocrinologist tests come back okay we will be back to square one , and im looking for some opinions of what to do next , and whether there are any other investigations you think would help

many thanks

Hello!

Welcome to Ask a Doctor service!

I passed carefully through your question and would agree with your doctor that checking your adrenal glands would be the next step to continue.

For this reason, considering the fact that you have also high blood pressure during these episodes, I would recommend checking these hormones:

- cortisol and ACTH levels
- urinary metanephrines levels for pheochromocytoma
- aldosterone and renine plasma levels.

If all these tests result normal, I would recommend a loop recorder, which is a device similar to the ECG Holter monitoring, but it can help monitor your heart rate for a longer time, compared to the ECG Holter and also catch possible cardiac arrhythmia.

You should consider also anxiety as a possible cause of all this clinical scenario, as it may lead to tachycardia and a higher blood pressure values.

You should discuss with your doctor on the above tests.

Hope to have been helpful!

Wishing all the best,
