Brief Answer:
One is unlikely to have Bladder cancer if no blood in urine .

Detailed Answer:
Hello
Thanks for query .You have not mentioned your age in your query .

Based on the facts that you have posted pain in bladder area that yu have without any blood in urine or other urological symptoms can not be due to cancer of bladder .

Blood in the urine (Hematuria) is the presenting symptom of bladder cancer .The enlarged lymph node in groin can be due to any infective pathology in organs draining in pelvic region which needs to be investigated .

Consult qualified Urologist for clinical and digital rectal examination and get following basic tests done to confirm the diagnosis.
1) Urine routine and culture.
2) Ultrasound scanning of abdomen and pelvis
3) Serum PSA.
4) Endoscopic Examination of Bladder (Cystoscopy)

Further treatment will depend upon result of these tests and final diagnosis.

Hope I have answered your query ,please feel free to ask if you have more questions,I shall be happy to help you
Thanks and Regards.

Thank you for your response. To follow up, I am a 64 yr old female, in good health. Swollen inguinal lymph nodes were the initial symptom starting about 3 months ago. They have fluctuated in size on both sides. Right side is more normal now, left side has one that is 1.0x 1.0 x 2.7 and another that is 1.9 x 1.8 x .07.A surgeon reviewed them and said to wait and watch til December. I have had a couple of UTIs in the last month and have taken antibiotics for them, each for a one week period, then they returned a week later (different type each time). Other than the lymph nodes swelling and the UTIs, my tests are all normal. Since late XXXXXXX I have had a colonoscopy, CT of abdomen and pelvis with and without contrast (no swollen nodes noted), transvaginal and external pelvic ultrasound and CBC blood work three times, all completely normal. I do have genital herpes (for last 40 years) with no other symptoms than the occasional blister and I have had them in the last couple of months due to stress. Beyond this, I am the picture of health. My apologies but the initial question space did not allow me to outline the facts in this detail. Please advise if this changes your initial thoughts that bladder cancer is not a likely consideration. In all UTI cultures, there has been no blood in the urine and no markers our of line but the bacteria of the ciprobachter feundii one time and enterococcus faecalis the other. I have had no fever, chills, etc. just bladder pain that is an ache, often quite painful, and pain on the left side where my nodes are. I am seeing a urogynecologist Wednesday but am concerned about bladder cancer and wanted to get some professional thoughts prior to my visit on the likelihood of bladder cancer or lymphoma. Could you please tell me what a PSA is that you recommended? Thank you, XXXXXXX

Brief Answer:
There is no possibility of Cancer of bladder at all.

Detailed Answer:
Hello
Thanks for follow up and highlighting the facts and investigations and reports that you have in detail. Considering the reports and and facts there is NO POSSIBILITY of cancer of the bladder at all.

As regards swollen lymph nodes they are mostly secondary to repeated episodes of infection (UTI) .However it is a normal phenomenon of the defense mechanism of the body to get draining lymph nodes enlarged and it is very little significance .

The pain in bladder area is due to chronic non specific infection of the bladder(Non Bacterial Cystitis) which is common in elderly females due to hormonal imbalance that happens after the menopause .

I can authentically rule out any possibility of cancer of the bladder .

The PSA is a test that is done in males only and my response to get it done was due to fact that you have mentioned your gender to be male in your initial query .

It is not to be done in your case .

Thanks and Regards.
