Change In Semen Parameters Like Colour And Consistency Should I Be Worried?

Question: Yes Dr Rynne,This is my delima , I need to know what kind of bacteria, STD, Infection , or Inflimation would cause a mans seman to change from the usual run of the mill, white milk like droplets to a totaly different color and consistency.
A color change and consistency that now more closely resembles the slimely , clear stringy sticky like consistency of Okra, ( If one has ever had the old southern dish called " Okra " ( Its kind of slimely, sticky
and stringy. (no smell ) this occurs after having sex with a female ( no condom ) however, this is the strange part, I have already gone to not only my primary doctors office, but gone into one of those big 24 hr emergency hospitals and just like my primary care doctors
full suite of Std tests even hiv tests, all came out negative. FYI approx 12 -15 years ago I had same condition, but back in those days the attending doctor or nurse simply gave me medication possibly antibiotics, cant remember but long story short, medication worked. now adays my primary doctor ( older female )
simply refuses to give me any medication. She simply says each time when I ask her for antibiotics, " medicines arn't benign, I can't percribe you anything when your tests for ,Stds return negative!!! ) As if this was not frustrating and strange enough, I also get an infection anytime I do use a lubricated condom, Any kind of lubricated condom. Now adays, most condoms sold at pharmacys appear to be Lubricated. Back in the old days as I previously stated
the attending doctor or nurse simply gave me medication and intire
incident addressed cured in a matter of 5 to 10 days. Now due to my
primary doctors refusal to perscribe antibiotics, I have entered new
territory as it relates to time That I have not gotten medical tretment. Never has my body had this particular malidy without being treated
for this length of time. I don't know what this condition may or may not be doing to my body since Ive never gone this length of time ( 1 month )without being treated. Sorry for being so long winded, but I am desparate to get treatment and answers for my condition. I see that you have been a family physician since 1968, Surely if any doctor would have come across a condition like mine, it would be you. Hopefully you have the informati!on I ve been seeking, Thank you Dr Rynne.

Hello and welcome to 'Ask A Doctor' service. I have reviewed your query and here is my advice.

Even though the change in semen parameters like colour and consistency could be a temporary issue with no major implications, however, a possible reason for the change in semen quality from being thick, grayish white to slimy, clear consistency could be a low sperm count. Therefore, firstly, I suggest a semen analysis to ascertain whether this could be a cause. Low sperm counts pertaining to an STD like chlamydia trachomatis (CT) is well known. Hence I would also like to know what all STD tests are carried out?Did it include a test for chlamydia detection? Even though CT can cause urinary tract inflammation, prostatitis and acute epididymitis, the infection can be asymptomatic in a large majority of males. Moreover, studies have shown a high prevalence of chlamydia infection in sexually active patients with abnormal semen parameters like changes in colour, viscosity and sperm count. Doxycycline or Azithromycin can be used to treat chlamydial genitourinary infections.

First of all thank you Dr Kakkar for your Assistance with regards to my medical issue in Question.
1) Here Are the STD tests performed approx 1.5 to 2 weeks ago.

Herpes Type 1 and 2 ( Negative )

HCV AB/Hep C Virus Ab.................NonReactiveg//Hg

HBsAg/Hep B Surface Ag.................Negative

HIV 1/2 Ab....................................Negative

N Gonorrhea Amplified Pro...............nagative

Chlamydia Amplifted Probe...............Nagative

Obviously The test thats missing is 1 of your possibilities and ( 1 of my guesses ) Which is ( Trachomatis (CT) I belive Dr Kakkar, you have found the missing STD Culprit.

I also believe This is true because I am one of those people that upone the slightist
smallest encounter of any type of infection, my body will immediately let me know that
something is wronge. (As also stated earlier I am also alerigic to All Lubricated condoms )

No different with this sexual non protected encounter. I notisted
a change in my Semen as previously extensively discribed within a mere 3 or 4 days,
Which I did not have these symptims prior to sexual incounter in question.

Even though I will do as you suggested about getting a semen Analysis. I fell you've
Already found the cause of my problem, especially When you pointed out the various
other simptons that (CT) can cause.

Please list the appropriate medication(s) for the Trearment of (CT) Just as you listed for Chlamydia above. Please supply answer and reply ASAP!!! I will be waiting with bated

No. I think the test for chlamydia trachmatis was done. Chlamydia amplified probe test is a Nucleic Acid Amplification Test(NAAT). It is done on a urine sample in males. It is negative in your case. Doxycycline 100mg, twice daily for a week or Azithromycin 1gm single dose is the drug of choice for chlamydia trachomatis in those who are either suspected to have the infection or infection confirmed by a NAAT. So, the next best step for you would be a semen analysis after abstinence for 3-5 days.

I will get A Semen analysis as you suggested. also i have practiced abstinence ever since
incident 3 -4 weeks ago.

I would like to know just how does ones doctor request you present a semen sample?
Is it like when a married couple can't conceive, and the doctor tells the man to take a
girly adult magazine into a private room with a small paper cup to return with a semen
sample?

Please doctor KaKKar, tell me what is or what are the semen collection protacols for a
male in this particular circumstance? thank you, Please reply asap!!!!

Hi.

The protocol is simple. Most centres already have such stuff available with them to help assist adequate sampling. A sterile plastic container is used to collect the sample. It would be provided at the centre itself. Abstinence for 3-5 days is good enough for an adequate semen sample.
