He can no longer bath, dress, or toilet on his own.

Vascular dementia as a disease per se does not decrease the life expectancy. However,patients with vascular dementia may develop parkinsonian features like rigidity of limbs(mainly lower limbs) and difficulty in walking. They have high risk of falls. Falls may cause head injury,which can result in death.
Some other complications like swallowing difficulty (may occur sometimes) and speech difficulty may appear.
Swallowing difficulty may result in aspiration pneumonia which can sometimes result in death.
These people are more prone to infections, which can be fatal sometimes.
Fluid and electrolytes abnormalities are also to be taken care of, to prevent adverse consequences.
