Hi,

Diarrhea in Crohn's disease is due to infection/inflammation. The purpose of treatment is to heal infection/inflammation.

The aim of Lomotil is to stop "hyperactivity" of the bowls, thus, stopping diarrhea. The real underlying problem is not resolved. That's why Lomotil is contraindicated in Crohn's disease.

I was hoping that your answer would deal more on whether or not we should be concerned about the incident of developing toxic megacolon with the use of Lomotil and not the answer you gave.

Hi back my dear,

Toxic megacolon is a serious complication of severe IBD; more common in ulcerative colitis and less common in Crohn's disease.

During Crohn's disease and other IBDs, there is an increased number of intestinal bacteria, which produce gas in excessiveness.

If a flare up occurs, and Lomotil is used, it might cause decreased bowel movements. This is very dangerous, as decreased intestinal movements would lead to accumulation of gas and bacteria, thus, leading to dilatation of colon, which is a very serious complication.

The main purpose of treatment is to reduce the number of gas-forming bacteria and infection/inflammation. Consequently, symptoms will improve without causing severe complications as megacolon.
