What Does This Ultrasound Report For Abdominal Pain Indicate?

Question: I am 46yo, 164cm, 55kg. Experiencing Abdominal pain. Irregular menstrual cycle with variable duration. Old blood present at times. Had an ultrasound. I am waiting to get in to see a gynocologist. Can some one please explain what may be the causes for the following report

Findings
Transabdominal and endovaginal ultrasound performed. Uterus is anterverted measuring 9.6x4.4x4.9cm. Endometrium is thickened at 14mm. Generalised heterogeneous echotexture throughout the endometrium which is thickened the fundus. There is a small cystic focus measuring up to 11x8x11mm.
The right ovary has a volume of 10.3cc and the left ovary of 3cc. 19mm heamorragic on right ovary might reflect a collapsing dominant follicle. Small trace of free fluid is present within the cul de sac

Comment
Heterogenous thickened uterous with evidence of cystic change. Probable collapsing dominant follicle or heamorragic cyst right ovary. Short term follow up is suggested. The

3) Do you have any H/O abnormal or heavy vaginal bleeding? How long is your cycle irregular?

Last menstrual cycle was approximately 14 days ago and lasted one day with minimal bleeding, the ultrasound was performed 5 days ago. I have not have had any heavy cycles. Cycles are very irregular, maybe 6 -7 cycles in past 12 months duration of cycle 1 - 2days, occasionally 3 days. Sometime the blood is almost brown/black in colour like old dried blood. Sometimes unusual odour. Recently (on one occasion only) i urinated what looked like pink watered down blood.
I Am not sexually active.
I have a 10 year old daughter. I have had 2 abortions pre the birth of my daughter

Hello Dear,

Thanks for additional info. I have reviewed your attached report. Followings are my comments:

1) Uterus is anteverted (i.e tilted towards bladder) - common in 75% women. But, it is slightly bulky as Normal size 7.6 X 4.5 x 3 cm.

2) You have undergone the scan on D9 (roughly) - during proliferative phase and it shows thickened endometrium (14 mm). Normal thickness is 6-8 mm. It could be due to endometrial hyperplasia (future risk of carcinoma), Poly Cystic Ovary, Endometritis etc.

3) Small endometrial cyst is also noted. It is a blood-filled growth, also called an ovarian endometrioma or endometriosis of ovary.

The symptoms of an ovarian cyst may be the same as those of endometriosis. Also, ovarian cancer risk is slightly higher in women who have endometriosis.

4) Right sided ovary is quite bulky and it shows small hemorrhagic or chocolate cyst of ovary, suggestive of endometriosis.

5) Small trace amount of fluid in cul-de sac is related with pelvic infection or current condition. It is nothing much significant.

TREATMENT: 1) Nonsteroidal anti-inflammatory drugs are frequently used for abdominal or pelvic pain. It should be taken with antacid.

2) Hormonal contraception with progesterone agent can be given to regularize cycle and control abnormal bleeding.

3) Definite - Laparoscopic surgical approaches is the best solution for excision of ovarian adhesions and endometriomas or chocolate cyst.

4) In most cases, endometrial hyperplasia is treated with removal of uterine tissue through dilatation and curettage (D and C).

I suggest you to consult with your gynecologist to proceed some other tests (like hormone profile, blood sugar, Hb%, CA-125 etc) before treatment plan.
