Meaning of your pathology report-
Dear, it means that the prostate which was removed then, had obturator fossa Lymph nodes removed on LT(left) and RT(Right) were Distant Lymph-node Area(dla).
You seem to have undergone prostatectomy surgery in 2003, and seem to have had Radical prostatectomy(RP)

Read and reviewed your updates in context to your worry about the Malignant Neoplasm of Prostate.
Gleason's-Score 3+3,6-indicates that the histological pattern is near to normal and is with discrete glandular pattern maintained overall but with invasion pattern seen .
Prognostically- your case is good.With meticulous care with Repeat PET MRI scans, the recurrence could be kept low in your case.But anxiety is more as the cancer is of aggressive but of intermediate grade(moderately differentiated type grade-).
Distant lymphnode Area - is also restricted below ext.iliac up to obturator right and left nodes and is not much advanced spread by TNM classification.

Explanation for you-
In your case your pathology report- suggests that your prostate cancer though malignant, has just crossed prostate -as it is moderately differentiated and yet aggressive with perinueral spread in the prostatic gland ,but with a cancer spread outside prostate gland but limited to nearby glands of obturator fossa and that the spread is not to distant locations-as to common iliac or internal iliac nodes and is confined more to nodes near and below external iliac nodes.

In short-Brief up-
your case is with good prognosis, though it is locally spread with moderately high risk due to its mild grade aggressiveness.

Prognosis-in your case.
And because of all these reasons, has better prognosis of getting controlled after surgery with a need for chemo / or radio / and / Hormone treatment modalities besides Radical Prostatectomy.
The beauty of this Gleasons-score - is that it combines histology patterns with TNM stage -arrived by MRI Scanning and Gives the prognostic assessment of a case after histopathologic studies of the prostatic cancer.
