Brief Answer:
diabetes mellitus, meet diabetologist /general physician

Detailed Answer:
Hi
Welcome to HCM

I had gone through your query, your fasting blood sugar level is too high, normal value is 60 - 100mg/dl
Your HBA1C level is also too high, which detects your past 3 months sugar level.
So according to your reports you are having Diabetes mellitus.
Consult your local diabetologist / general physician at the earliest to get complete check up and for the prescription of insulin injection. as your blood levels are high you may need to start with insulin injection for few days and later you can shift to tablets (combination of metformin and glimipride).

Prevention
1 By maintaining a normal body weight, engaging in physical exercise, and
consuming a healthful diet.
2 Dietary changes known to be effective in helping to prevent diabetes include
maintaining a diet rich in whole grains and fiber, and choosing good fats, such as
the polyunsaturated fats found in nuts, vegetable oils, and fish
3 Avoid red meat

thank you
