Brief Answer:
Urethroplasty would be correct treatment.

Detailed Answer:
Hi XXXXXXX
It seems like a chronic stricture or narrowing of the urethra by the MCU/RGU report.

I would like to know the age and problems faced by the patient.
If he does not have severe symptoms I would suggest to leave it alone for the time being and see how it recovers.

If it has been there for a long time and is troubling him then a Urethroplasty would be required.

So, do let me know about his age, symptoms and since when is the problem. Also do let me know whether he has Diabetes or not.

But the treatment offered seems correct and so was the investigation carried out. So do not worry. Take care and I will look forward to hearing from you.

Brief Answer:
You should get Urethroplasty by a good Urologist.

Detailed Answer:
Hi XXXXXXX
If the symptoms are as severe with pain and inflammation then I would suggest you undergo a Urethroplasty.
Its good that there is no Diabetes then chances of infection and delayed post operative recovery are much less.
The symptom of insensate ejaculation might be due to the stricture in the urethra but its not very sure. I would advise you to undergo the procedure as this might relieve the problems you are facing.
Do let me know if there are any other queries I can help you with.
Take care,
