Question: LS SPINE XRAY W/OBLIQUES
History:Back pain
Technique: AP, oblique and lateral views of the lumbar spine were obtained.
Findings: There is a transitional vertebra at the lumbosacral junction is designated as a partially sacralized L5 segment. There are rudimentary 12th ribs Alignment is preserved. Bones are normally mineralized without a fracture.
Pedicles are preserved. There is no focal osseous lesion.
Disc spaces maintain normal height. There is no osteophytic ridging.
There is mild facet arthrosis at L4-L5.
SI joints are not widened. There is no calcified soft tissue mass.
Impression:
Transitional vertebra at the lumbosacral junction designated as a partially sacralized L5 segment.
Mild facet arthrosis L4-L5.

Should I see an orthopedic or a neurologist to follow-up with? Do you think an MRI would show more?

This suggest you are having lumbar spondylitis

Hello,
I have studied your case report and it is showing following things.
1. You have a congenital spine malformation which is very common and can cause back pain.
2. Between fourth and fifth vertebra there is degenerative changes, there can be a disc herniation. it can also cause back pain.
These changes are not very specific though and can be present in normal person also. I would recommend you following.
1. Get MRI of the spine to confirm disc herniation.
2. Avoid heavy weight lifting, driving or any jerky activities.
3. Back muscle strengthening exercises will help you.
4.Steroid injection can be given in the spine.
I hope these measures will help you. Let me know if there is any other followup questions.
