What Does My Semen Analysis Report Suggest?

Question: Question about sperm count. On the first semen analysis, it was quoted as <1million sperms/ml. The next two (with 3-day gap between each) showed 0 sperms.
Then a testicular biopsy was done and the report said sertoli only. The doc said I am infertile. Is there any way I can change that and have my sperm count improved? Appreciate any help in this regard.

Hello, XXXX. Very sad to hear about your concern. I am Dr Arun Tank & I am infectious disease specialist I am going to answer your concern.

-As you have said sperm count in semen is zero also testicular biopsy showing sertoli cells which is the premature cells of the sperm cells. My view is you must be diagnosed about the underlying cause. The problem you have mentioned is mostly observed when there is duct blockage. But I do not know exactly because I dont know your proper history. Did any doctor had given you underlying cause? I want to know about did you have undergone any USG? Did you have undergone any laboratory test?

-If the duct blockage is the cause we can search where the duct is blocked and accordingly we go for management.

-You also think in some of the preventive aspects. I suggest you should pass stress free period before undergoing semen analysis. Take good fruits and antioxidants. See some of the good porn movie.

I was actually stressed out while giving samples due to several other reasons. I did not undergo any USG or lab test. It was just 3 Semen tests and then a biospy on right testicle. I am attaching all of those reports. Kindly advise. Thank you very much .

Hello Hbase19-all, Sad to tell you that you have been diagnosed as sertoli cell only syndrome.

-It is chromosomal mutational (deletion) syndrome. Sad to say you that it has no cure. Only option left with you is to aspire the sperm which is viable and inject it into mature ovum. This way fertilization would take place and your problem will be solved.

I have following questions -
1. Is the suggested solution same as Donor sperm?
2. Is it not possible for finding a viable sperm in me at all? The first report showed a non-zero sperm count. Is that something which can be hoped upon?
3. Is there any alternative at all for my problem

May I please also ask you for a good place to seek further help in Hyderabad??

-Yes you can go for either donors sperm or yours. In your first report there are some sperms present. Which means sperms are developing in your testicles. You can harvest such sperms from your testicle and you can use it. Though it is some what tough but you can go for it.

-In the second option you can select the finest donor and take their sperms for artificial insemination.

-There are no more alternative to it, you can also opt for adoption.
