Question: 44 yrs male has history of Fever and Investigated found Hyperthyroidism her Anti thyroglobulin Antibody – 89.50 and Anti Nuclear antibodies positive 1:80 and Anti TPO Antibody < 10 . Isotope scan showing solitary nodule .in 2015 . Two years of followup with TFT , are normal , no medication after 2015 . However CTMT test was positive at workload 13 METS for Ischemia What can be the chances of Cardiac complications , and prognosis and suggested treatment any further investigations .

Stress test is only 60% sensitive to rule out Coronary artery diseseae. In lay man's language it's among one of the baseline test of cardiology.
Protocol should be if TMT is postiive go for 2d echo.Combined with Echo the sensitivity to rule out coronary artery disease is 85%.
However as you have issues with thyroid you are a suseptible candidate for coronary artery disease.Hence i would advice you to go for Stress thallium directly.Sensitivity of stress thallium is 97%.A thallium stress test is a nuclear imaging test that shows how well blood flows into the heart while you’re exercising or at rest.During the procedure, a radioisotope is administered through an IV. The radioisotope will flow through your blood stream and end up in your heart. Once the radiation is in your heart, a special camera called a gamma camera can detect the radiation and reveal any issues your heart muscle is having.Its a very safe noninvasive test and takes about 2 hours.
Prognosis and treatment depends on findings of stress thallium.

Follow up with reports.
