What Does My Child's Sonography Report Indicate?

Question: Dear Doctor,
My wife is 26 yrs of age & in her recent OBST colour doppler Sonography the child is having 2 loose loops chord round the neck. Do we have to go for ceaserean only or can we still try for normal. Last OBST sono was done 10 days back & today repeated but the loose chords still visible. Pls suggest..

Caesarean is a safe option but you can try for vaginal delivery also.

Thanks for writing to us about it.

I can understand your concern.

With your history I can understand that your child has two cords around her neck and your wife is suffering from high blood pressure taking medication for it and it is under control.

These are my opinion in her case-

-As such, two cords around baby's neck is not an indication of caesarean section, but to be safer side usually caesarean section is recommended.
-In vaginal (normal) delivery -during delivery there is pressure on baby's neck due to pull on cord so baby heart rate changes and he may go into asphyxia due to prolonged labor.

In her case, as the baby has two cords round her neck and she is suffering from high blood pressure and this is her first delivery in my opinion caesarean section is the better option.

But if you desire to go for vaginal delivery it should be done under an expert supervision and facility of NICU should be there.
Monitor baby fetal heart rate and fetal movement and progress of labor during delivery.

Thank you so much for your valuable opinion. Just tell me what do you meant by NICU?? & as you advised, we were also of the same opinion that atleast we should go for normal delivery first & in case if there is any indication of changes(deterioration) in child health or if the normal delivery does not happen during that time then Doctors can go for ceaserean. So my concern was only that can we take the last minute decision during the labor? We are currently visiting the nearby pvt. nursing home where i doubt that they have the advance facilities. so shud I visit a full fledge hospital during delivery or can suggest some good (experienced) expert Gynaecologists in XXXXXXX where i can visit during labor?

NICU means the neonatal intensive care unit in which expert team of pediatricians and fully equipped unit for neonatal care is available.

Yes, if you desire to try for normal vaginal delivery you should go to fully equipped hospital where all the facilities for neonatal care is available.

In XXXXXXX you can go to any multispeciality hospital like FORTIS etc. where you can get all the facilities under one roof.Choose the hospital which is near to your place).

My wife has completed 39 weeks 2 days today..should we have to wait for the labor pain or does she need to get admitted now?? Our current gynaec said to visit twice a week & immediately in case of any blood or water discharge or in case of pain or stoppage in movement..currently the movement is there & rest everything is normal & as per the repeated doppler done day before yesterday, child is in good condition apart from the loose nuchal chord.

Sorry for my delay in response.

As your wife is already completed her 39 weeks of pregnancy, in my opinion, she should get admitted in hospital and she should get induced for labor.

I am saying for induction because your wife is having high blood pressure and the baby is having two cord round neck, so induction is a safe option rather than wait for labor pain.

As your treating doctor's opinion you should wait for normal labor pain as your baby is fine in ultrasound, you can consider her opinion also.
