Brief Answer:
Granular Casts and Mucus threads are detected in urine and never in blood.

Detailed Answer:
Hello
Thanks for query .

I shall be happy if you can post me more details to help me in making precise comments
1) Do you have any urinary symptoms like burning urination ?
2) Are you diabetic ?
3) Have you ever done renal function tests to assess the kidney function ?

Based on your statement that you have stated I would like to mention that Granular Casts and Mucus thread are detected in urine and never in blood .I think you have mistaken it to mention blood instead of urine .

Finding granular cast suggests some kidney disease of Tubules .However granular casts needs further tests to rule out any kidney pathology

Consult qualified Nephrologist or Urologist for clinical assessment and get following basic tests done to establish diagnosis .
1) Urine routine and culture.
2) Ultrasound scanning of abdomen and pelvis
3) Serum PSA.
4) Renal function tests

As regards presence of mucus threads it is common to find mucus threads in urine even in urine of a normal man.It is significance when their number is large which usually suggest infection and dehydration .Taking antibiotics after proper test like urine culture and drinking more water resolve the issue

Hope I have answered your query ,please feel free to ask if you have more questions,I shall be happy to help you
Thanks and Regards.
