What Does Bleeding During 12 Week Of Pregnancy Indicate?

Question: Hi
LMP of my wife is 02 XXXXXXX 2015. She has bleeding in the 12th Week on dated 24th Mar. USG was done on same day and report shows placenta is posterior and extended in lower uterine segment just touching the internal os with marginal separation.
NB & NT scan & Double Marker was done on 30th Mar. The results are: 1. The nuchal lucency is 1.9 mm and normal. 2. Nasal bone is visualized (2.1 mm) and appears normal.3. Placenta is posterior in location, low lying lower end of placenta almost 1.2 cm from the internal os. A small hypoechoic area of size appx. 11X 6 mm is seen along the lower edge of placenta. Cervical length measures appx. 3.6cm.
4. CRL length is 61.9 mm corresponding to 12 weeks 4 days of gestation.
The results of Double Markers are: 1. Free Beta Human Chorionic Gonadotropin- 48.60 ng/ml
2. Pregnancy associated plasma Protein A - 1.35 mIU/mL (method for both: Chemiluminescence)
Mutiple of Median (MOM) values (Foetus 1):
Free BHCG MOM- 1.21
PAPP-A MOM - 0.45
NT MOM- 1.19 Method for all values : Chemiluminscence
Risk Calculation (Foetus 1)
Trisomy 21 screening with NT 1: 1374
Trisomy 13/18 screening with NT <1: 10000
Doctor has advised for the medicines, Duphaston twice daily and Susten 200 intravaginally at bed time daily.
My question is whether these reports are normal or required further any test/investigations and is it safe to take both together Duphaston and susten 200 during pregnancy.

I have read the detailed report carefully. The report is normal. It shows a low lying placenta that can cause a mild bleeding occasionally in case of exertion.
For proper placental attachment progesterone supplements like Susten are given till 14 weeks of pregnancy. She can continue taking Susten as advised. Both Duphaston and susten are not necessary. It is best to stop Duphaston after consulting her gynecologist.
The chances of any congenital malformation are minimal. No need to worry.

Thanks for your valuable advice.
is there any further need of doing triple/quadruple marker test.
Is taking both susten 200 intravaginally at bed time and Duphaston twice a day has any side effect on the baby. These both were started from 13th week only.
Is the bleeding in 12th week can cause any abnormality in the fetus.

Thanks for writing again.
Triple and quadruple test have their own significance. Even if double marker test shows a low risk still it is best to go for triple and quadruple test after consulting your gynecologist.
Taking both susten and duphaston will help in proper placental attachment. It will help in stopping the bleeding that your wife had. She can continue these for 2 weeks and then consult her gynecologist in case further continuation is needed.
A small bleeding at 12 weeks will not cause any abnormality in fetus.

You had given very valuables advices, we will visit our doctor shortly.
My wife had bleeding only once in 12th week and thereafter minor blood spot in next 2-3 days. At present, since almost 20 days, there is no bleeding or spotting at all.
Is there any side effect of susten and duphaston on fetus.
is there any requirement of taking any supplement with susten and duphaston to lessen their side effects.
Our Doctor has asked for genetic counseling with genetic specialist. I and my wife's family has no history of abnormality in babies. NT scan and double marker results are normal. According to my view, i think there should not be any need for genetic counseling.
What is your opinion in this regard.

If there is no bleeding or spotting for last 20 days then intake of progesterone like Duphaston and susten are not needed. These can be stopped gradually after tapering the doses.
Abnormalities of the genitourinary tract are found in a baby whose mother had taken Duphaston (dydrogesterone) from week 8 to week 20 of pregnancy. That is why it is not recommended for use if there is no bleeding or decreased progesterone levels.
Any supplement cannot reduce the side effects. It is better to stop these after reducing the doses gradually.
With a normal double marker test and no family history of genetic abnormality there is no need of genetic counselling. It can be needed if both of you have different blood groups in case of Rh factor- negative or positive.
