What Do White Blood Cells In Urine With No Symptoms Indicate?

Question: I am 67 yo male. I am a Type 2 insulin dependent diabetic. About 6 weeks ago I had a routine urine analysis. I had no dysuria, frequency or hesitation. I had Cloudy urine with 50-60 WBC. Urine culture was negative for bacteria, I was treated with cipro 250mg qd for 2 weeks ands repeat UA after therapy was negative. Renal US was negative for hydronephrosis or stone in late January

As part of routine physical once again urine was positive for wbc 40-50. Again no symptoms. What are your thought. I am allergic to PCN and Sulfur?

Hello,

no symptoms means no treatment according to current guidelines about asymptomatic bacteriuria. Whenever asymptomatic bacteriuria is accompanied by pyuria (increased number of white blood cells) no treatment is indicated.
The rationale is that medical studies have shown that treatment does not confer any benefit because most of the times the patients will develop asymptomatic bacteriuria again in the near future.

So the answer is that there is no sign of allergy or anything else. You just have to accept that your urine will probably have white blood cells and bacteria. If this condition does not affect your bladder (by causing symptoms) then you shouldn't worry too much.
