Suggest Treatment For Recurrent UTI After Marriage

default
Posted on Mon, 21 Sep 2015
Question: I am married since November 2014. I have UTI for past 9 months and it's recurrent. I have taken cefixime for 12 days, ciprofloxacin for around 7 days and intravenously Ofloxacin 7 days.. As it was recurrent, I stopped taking antibiotics and went for homeopathic medicines. My burning sensation and urethral pain was better aftr that. I recently checked my culture report and urine complete report. I see heavy growth of e.coli more than 10^5 cfu/ml urine isolated in culture and pus cells/epithelial cells are occasional in the urine complete report. My protein count is 10 in urine

Due to lot of antibiotics, I have got GERD and my conditions are better now.

Is there any permanent solution to recover from this recurrent UTI?
I am planning to get conceived.

Should I consult a Nephrologist / Urologist / Urologist? Kindly suggest me a best solution for my problem.

Brief Answer:
Renal imaging is mandate in your case.Relevant data provided.

Detailed Answer:
Hello, Madam.

I understand your concern.

I have gone through your uploaded images as well.

You seem to have recurrent and resistant cystitis(UTI).
Every UTI is associated with some degree of protein loss in urine.

The latest report(culture and sensitivity)shows the presence of bacteria(greater than 100,000 CFU/ml) in the absence of symptoms(or even reduced symptoms0.This is called Asymptomatic bacteriuria.

This can pose a threat to your baby.

Hence, it is very important to find out the cause of recurrences in spite of treatment. The common causes are-

1.Frequent sexual intercourse.

2.Infection in the husband-It is important to treat your husband simultaneously.

3.Coexistent fungal infection.

4.Undetected diabetes-This warrants screening in your case.

5.Stones in kidney, ureter or bladder-This stress the need for CT scan abdomen in your case.

6.CBC-complete blood counts to detect any immunological deficiencies.

Based on the test results-therapeutic options can be carved out and permanent cure can be offered.

I advise you to consult both Nephrologist and a Urologist once the above investigations are meted out.

The addition of antibiotics like Fosfomycin and Fluconazole can be an important part of management. Please check with your doctor if he shares my view and can prescribe them to you.

I glass of cranberry juice and 1 liter of water within 30 mins of sexual activity largely help to prevent UTI's to a great extent.

Post your further queries if any.
Thank you.
