What Do These MRI Findings Indicate?

Posted on Mon, 7 Dec 2015
Question: I have attached two pdf's. I must warn you one is a picture of my so called Hemorrhoid along with an MRI picture of my pelvis. The other is an MRI report that states prudendal nerve entrapment. This tear in my rectum occurred during a prostate biopsy. The tear is very tender and painful. My questions are: 1) should this be seen by a doctor and 2) could this have cause my entrapment because I keep the area very tight because of the pain. Notice the MRI picture and the muscles on the left are all kind of torn up. This is very, very, very painful. Burns all the time.

Brief Answer:
Surgical intervention is required.

Detailed Answer:
Hello, Sir.

I can certainly understand your concerns.

I apologize for the delayed response owing to my hectic work schedule.

I have gone through your query and attached images/reports in detail.

Yes, certainty the hemorrhoids are directly as a consequence of Rectal tear.

The MRI clearly demonstrates inflamed muscle and nerves.

This condition certainly needs a surgical consultation and an intervention.

The hemorrhoids seem to be of stage 2, but requires per rectal examination for confirmation.

To conclude early surgical repair is indicated.

Post your further queries if any.
Thank you.

Further delay not encouraged

Detailed Answer:
Hello, Sir.

I can certainly understand your concerns.

I do not think it's too late for the surgical procedure.

It's important that you assess your condition further and at the earliest via a fresh MRI scan if feasible and report to your surgical physician.

You seem to have entrapment neuropathy.

This needs a procedure called decompression and release.

For hemorrhoids either hemorrhoidectomy or stapling is done.

Please check with your physician if he shares my view and recommend these procedures to you.

Thank you.

Brief Answer:
Surgery rare, but not uncommon

Surgery for pudendal nerve entrapment even though rare but not uncommon.

Surgery is usually the last resort to manage the chronic pelvic pain and parasthesias.

Radiofreqency pulse wave therapy can be initially offered failing which surgery can be undertaken.

In my ecological conditions, we had few successful surgical outcomes for decompression in our patients.
