Question: Hi, I had a few red, itchy bumps on my scrotum and penis that multiplied and got bigger, but are staying contained to just those areas. Thought was just a bad case of jock itch, so have been using Lotramin for 2 days now and it seemed to help dry things out a bit....though now the bumps on my scrotum have pretty much turned into a red, thicker skin over my entire scrotum...

You said that you have erythematous papular eruptions on penis and scrotum. And you have itching also. I would like to ask you few questions...
- from how many days you have such problem?
- do you have similar or different lesions elsewhere?
- do you have any other disease?
- what is your work?
- is there any family history of similar or other disease?
- past history of similar or other disease?
- blood reports if done?

Please answer and post the photo of the affected parts.

Follow up: Dr. Ilyas Patel (43 hours later)
- Red bumps and itching started last Sunday, lasting for 7 days
- swelling began Friday and increased to cover scrotum Saturday (so now 3 days)
- itching has gone down dramatically as the swelling and thickening of skin has set in
- for last 2 mornings, penis has swollen folds...but yesterday this swelling on penis had gone by the afternoon. But woke up with penis showing swollen folds again...seems to be diminishing as day goes on
- but scrotum skin is still very thick and red
- today is starting to show white flaking of skin
- Lesions were isolated to scrotum and penis
- now mostly red, rough skin...slight soreness, but very low to almost no pain

- I have no other diseases, though have had somewhat sensitive skin...nothing that required medical care (other than athletes foot I suppose?)
- no family history of related illness
- work = web development, office and computer boound
- environmental conditions: I had spent last Saturday swimming in a local river and then Sunday swimming in the ocean before I noticed first signs of the rash Sunday night.
- no blood reports have bee done yet

Went to a clinic yesterday and thought maybe the rash came and then I had an allergic reaction to the Lotrimin Ultra I used to treat it. Since I started using Lotrimin Thursday night and the swelling started Friday, progressing over the weekend. My last application of Lotrimin was Saturday morning (9:00 am)...but swelling is still there as of this morning (Monday 11:00 am)

Hi,

Welcomw back..

I read history and saw the photo you posted. You had swimming in river and ocean on last saturday and sunday. I think, most probably you may be having irritant contact dermatitis on genital and scrotum. The culprit agent may be in river or ocean water. It may be some pollutant or chemical or it may be some insect responsible for the disease on scrotum and penis. It may be called scrotal dermatitis.You consult dermatologist for firm diagnosis.

I generally prescribe in patients having simmilar problem ...tab. methyl prednesolone in tappering dose along with antihistaminic like levocetirizine. And advise to apply mild steroid cream. If needed,antibiotics may be taken for short period. Avoid contact with soap or other irritants. And do not go into river or ocean till you get good result.

I hope you might be alright after few weeks treatment. I feel..you got my answer.
