Brief Answer:
Admit him to hospital

Detailed Answer:
Hi

Reports of your father suggest that he is suffering from advanced renal failure. serum creatinine mean values above 6 indicate that kidney function has become very bad . medications prescribed by the doctor for control of the effects of damage kidney on the body but we need to find out why the Kidneys are getting damaged .

for this we need to know the ultrasound of the Kidneys and urine routine examination reports.
This will help us in finding out the treatable cause for renal failure. in any case with the present situation your father should be admitted to the hospital immediately and all efforts should be made to find out a reversible cause for renal failure. If the patient is developing breathlessness then there may be need for him to undergo hemodialysis also. it is not safe for you to keep him at home with such level of Kidney Disease

best wishes.
