Question: hello Doctor,
i m 31 yrs old and married for 4 yrs nw. for last 4 yrs we hd no plan of having a baby. bt nw we are planning 1 . we r trying from last 3-4 mnths bt no success. i hd consulted a doctor first tym last yr in april when for 1.5 mnths my periods were delayed. after all failed pregnancy test,it came out that i had a cyst nd then later poly cyst. i consulted our family gyno. she askd me to take HSG which showed left tube blocked. bt prescribed other fertility tabs for conception and antibiotics for ny kind of infections. she continued few medicines like... susten 200 mg from 18th day of memses, fertipreg f,cabegoline 5mg etc.
last month she asked me to take goodova from day 3 for 5 days and then take sonography on 13th day. in sonography i had 2 eggs on rt ovary of size 18*18 mm and 4 eggs in lft ovary of similar size bt i had complained abt vaginal dryness during ovulation. she had prescribed medicine for it too. bt no result.
this month she has suggested me fertyl m-25mg from day 3 and progynova 2mg from day 8 of menses for 5 days each. and sonography on day 12 or 13.
i want to know if i m getting the right treatment and what are my chances of conceiving in this cycle?

I have gone through your query and understand your concerns. Let me assure you that the treatment is on the right track and the medicines are proper. However, it might still take a number of cycles for the conception to occur. You need to have sex with your partner on alternate days during the prospective period of ovulation. The time of having the sex needs to correspond with the ovulation as the ovule remains alive for only 24 hours and the sperms need to reach the ovum when it is in the fallopian tube. So, a lot of factors need to fall in perfect alignment for the pregnancy to occur. You would need to be patient. We can only hope that things fall in place as soon as possible, preferably in this very cycle.

1.Kindly suggest some treatment(can b home based also) or some exercise for egg white cervical mucous.

2. Can blocked fallopian tube be opened with any of these medicines or how and when will my tube be opened?

3. Is Fertyl M 25 mg for ovulation or egg formation? Doctor askd me to take for 5 days bt the chemist told it should be taken for 1 mnth. pls clarify what is fertyl M 25mg tablet actually for?

4. if in this cycle also we dont succeed then what would be next course of treatment?

5.What are the probable reasons that i m not able to conceive yet?

Detailed Answer:
1. Ovulation automatically changes the hormonal balance and the cervical mucus gets diluted and removed. Exercises will not help in its removal.

2. There is a test called hysterosalpingogram which indicates the patency of the fallopian tubes. Unless the test shows blockage you need not worry. If apprehensive, get the test done.

3. Fertyl M contains clomiphene, an ovulation inducing agent. It should be taken for 5 days. It is an irony that chemists get to play the role of doctors!

4. If this cycle does not become fruitful, a careful correlation would be needed about what could have possibly have gone wrong. In the next cycle those factors need to be taken care of and another attempt needs to be taken.

5. It may take 2-3 years for a normal conception to occur when there are no noticeable problems. The first step would be to clear the basics. Evaluation of both the male and female counterpart is required. Semen analysis in the male on one hand and hormonal assay in the female on the other, are the first steps. The next step would be hysterosalpingogram. Folliculometry at around the possible time of ovulation is the subsequent step. Each step provides valuable clues. You would need to confirm that nothing seems wrong in the mentioned steps. If that is the case, an in depth look is required. In short, it is not possible to randomly state the cause without being methodical.
