Posted on Mon, 18 Aug 2014
Question: I had a milky discharge and was given a azithromycin and suprax. Three days ago. Now the milky discarge is dark yellow or brown. When will my penis stop discharging this fluid.
No pain when I urinate. If my penis becomes erect. I have a slight itchy or tingle below the tip of my penis. I am discharging small amounts of fluid hour after I urinate. Took my medicine 72 hours ago.

Brief Answer:
Gonococcal urethritis

Detailed Answer:
Hi, thanks for posting your concern in the HCM.

From your symptoms, I think you have developed urethritis, most probably due to gonococcal infection (GU).
My opinion is your doctor has correctly prescribed those two antibiotics for your condition.
You will stop having the discharge as soon as you get cured from the infection.

For the time being, my recommendations are-
1. Please maintain good personal htgiene.
2. Avoid multiple sexual partners and unprotected intercourse.
3. You and your partner should have tests to rule out STIs including HIV. NAAT may be done, if available, to specify the diagnosis of GU. Tests should also be done to rule out syphilis, chlamydia and HIV infection.
4. A complete blood count and CRP are needed to rule out systemic infection.
5. Urine for routine examination and culture-sensitivity may be done.
6. A calcium alginate swab may be taken for urethral discharge to perform culture sensitivity testing.
7. Good oral hydration should also be maintained.

However, culture reports may come out as negative, as you are already on antibiotics.
You and your partner should also consult a venereal disease specialist or visit the STI/HIV clinic at the earliest.

There are several complications of urethritis in males as well. Your doctor will rule out those by clinical examination and if necessary, appropriate investigations.
Again, I should tell you that yellowish brown urethral discharge , itching at the tip of penis- all occurs due to urethritis and will be resolved as soon as the infection is gone, unless there is any complication.

It has been four days since I took the above mentioned meds. Now the Milky discharge is dark brown or has a hint of blood in it. I am not leaking like a water hose. However after I sit down or go idle for a while I will have a small drop of ooze at the tip of my penis. No pain, but if my penis becomes erect I will have a slight tingle about one two inch below the head on the main vain or vessel. Is this normal for the milky to become bloody and is it normal to have a discharge four five days after taking the meds.

Brief Answer:
Gonococcal Urethritis

Detailed Answer:
Thanks for your reply.

My opinion is- it is not at all normal for the milky discharge to become brownish or blood-stained. You most probably have got urethritis and that too probably a complicated one. And that can happen despite having antibiotics. The organism might have not been killed or might have caused some complications.

But, the symptoms you are telling me, definitely suggest a provisional diagnosis of what I have already mentioned.

Therefore, please follow the recommendations I have already provided in my previous reply in this thread. I am confident that this will help you.
