Brief Answer:
Therapeutic circumcision is the best plan here

Detailed Answer:
Thank you for asking

I read your question and i understand your concern. Patches under foreskin reducing pulling back and retraction of foreskin is indication of prepuce adhesion secondary to penile infections like phimosis, balanitis or paraphimosis etc. FUngal infections like yeast can also be the trigger.

You need therapeutic circumcision for the permanent and effective relief and also to prevent complications like penile curvature issues, long term complications of malignancy of penis and also bad coital life.

Conservative options like manual reduction, osmotic reduction, hyaluronidase application and vertical incision are some of the options but chance sof recurrence are very high and the issue persists.

Talk to your urologist and let them decide what is best for you.

I hope it helps. Take good care of yourself and dont forget to close the discussion please.
