Posted on Wed, 29 Apr 2015
Question: I developed an itchy red rash which started around my neck then proceeded to upper back, lower back, buttocks, and outer thigh areas. No itching or rash on inner thighs, left arm, lower legs. After seeing the er twice I was put on itch cream and medication to heal rashes. Now rash went away but I keep having these phantom itches that are in the same places the rashes were. Which re reddens and they go away, only the itch continues!!!! I have hypothyroid and have trouble losing any weight. Lastly...I find I can go all day at work and don't itch at all...but it starts again at home and especially at night and first thing in the morning?

Brief Answer:
I suggest possibility of Urticaria; Oral daily antihistamine e.g cetrizine.

Detailed Answer:
Hello. Thank you for writing to us.

I have gone through your query and I will try help you with your concern.

From your description these rashes do seem as if they are hives/ urticaria.

The rashes in urticaria present as welts (raised/swollen lesions) that are itchy and may be distributed widely on body.
The welts may last for just a few hours (4-6 hours) i.e they are evanescent and may appear again after they disappear. This may go on for a few days.

Underlying cause for a recent onset urticaria could be allergic e.g food or food preservatives; drugs e.g antibiotics/ painkillers; pollen etc.
Common infections like a viral common cold or flu may also be associated with urticaria.
Most of the cases of recent onset/ acute urticaria resolve within 2 weeks.

Urticaria, specially chronic urticaria, is more common with underlying thyroid.

Treatment is symptomatic i.e daily oral antihistamine e.g cetrizine 10 mg once daily for a few days.

Topical soothing lotion or an anti-itch cream can be used over the rashes.

Brief Answer:
Start with cetrizine

Detailed Answer:
Hi.

Thank you for the details.

Hydroxyzine is an antihistamine but it is short acting and also sedating.

I suggest you to take oral cetrizine 10 mg twice daily for a week. It is available OTC.
Cetrizine is long acting and therefore would be more effective.

You may continue with the anti-itch cream.
