What Causes Upper Abdominal Pain, Weight Loss And Sleeplessness?

Question: I have pain in upper abdomen(LHS) for the last 4-5 months. I have taken various medicines like SMPRAZ HP Kit, Nexpro, Nexpro RD40 and for the last 1 and half month Nexpro-L. Pain was vanisehed when I was taking Nexpro-L. After this, there is slight pain or irritation once again. Bowl is normal, no major changes as such. Night sleep is disturbed. Weight loss 2-3 kg in last 4-5 months. Taking diabetese medicine for the last 7-8 months. What should I do now?

Brief Answer: medication and precautions will help. Detailed Answer: Hi, Thanks for writing your query. After reading your post and seeing the attached reports, it seems that you are having Gastro Esophageal Reflux Disease. The medicines you are taking are correct. You should follow some precautions like :- 1.Avoid being empty stomach for long periods. 2.Avoid spicy and oily foods. 3.Avoid mental and emotional stress. 4.Avoid smoking and alcohol, if any. 5.Avoid self medications, especially pain killers and steroids. 6.Drink lots of water daily. 7.Eat light meals prepared under hygienic conditions. 8.Curd can be helpful. Weight loss can be seen as intake of food is slightly decreased due to pain. Try to get sufficient sleep. This will help in keeping you stress free. If pain persists, you can take antacid in gel or liquid form as this will form a protective coating over the damaged mucosa of the gastrointestinal tract. I hope this is helpful to you. Sir i) Do I need to undergo any other testing like CT Scan etc ii) I have mentioned various medicines which I have taken like SOMPRAZ HP Kit, Nexpro 40, Nexpro RD 40, Nexpro L. Tab Adiza(10mg) and Tab Stelbid have also been suggested. Which medicines should I continue and for how long and what should be the dosage? iii) You have mentioned to take lots of water. How much? regrads

Brief Answer: You can continue Nexpro L. Detailed Answer: Hi, Thanks for your response. You should continue with Nexpro L for about 4-6 weeks. Since endoscopy has confirmed for GERD, other investigation may not be needed at present. About 8-10 glasses of water daily is sufficient. I hope this answers your query. Sir Thanks for your response. i) I have taken NEXPRO-L twice a day for a month and once a day for 2 weeks. What will be the dosage now? ii) My mouth gets dried up very frequently, what is the possible reason and what should I do? iii) Sometime there is slight pain in my hands and legs. What is the possible reason and what should I do? iv) My sexual activities have also reduced. What is the possible reason and what should I do? Regards

Brief Answer: your symptoms can be due to medicine. Detailed Answer: Hi, Thanks for providing more information. After reading your post,it seems that your symptoms are due to levosulpiride present in Nexpro L. You can stop the medicine and whenever required take Nexpro (Esmeprazole) with Domperidone 30mg. Duration depends upon the severity of your symptoms. If you are not having continuous pain, you can take the medicine as and when required. Follow the precautions mentioned in my earlier post and you will need less medication. Since your symptoms are medicine induced, they will start to improve gradually after stopping Nexpro-L.
