What Causes Severe Pain In The Head And Depression?

Question: Hi Doctor,
This is harikumar,35 yrs,working in siemens.
From six months, I was feeling pain in my head.
When I talk about office issues, and with new colleagues, I feel stressed depressed,and worried. I feel neck pain also. and sometime I feel unconscious thinking all these works.Nowadays, even at home also I started feeling this.
kindly advice what to do.

Below are my answers for your questions

where exactly do you experience headache? the type of pain- throbbing, pulsating, tightening?
yes, the pain is left side of my brain, throbbing and tightening type and sometimes I may not look down because of neck pain. I simply sit and relax ,until the pain calm down.

- do you experience heaviness in head while bowing down?
yes I experience heaviness when I bow down.

- at what time of the day is the headache more prominent?
Usually, I get this pain just before lunch time (around 12pm).
After lunch I feel good partially.

- do you have any other conditions like high blood pressure, high cholesterol, history of migraines in your family, associated blurring of vision, vomiting?

I have cholesterol, blurred vision and I am diabetic but not using medicines.
As per family history, we have diabetic only.

Hello again,
Thanks for writing back,

From the description that you have provided, I would list down the possible diagnoses:

- Tension headache
- Headache due to sinusitis
- Migraine

In order to rule out sinusitis, I would like to know if you have symptoms of frequent colds, nasal block or discharge, tenderness while touching the forehead or cheek bones.
In that case, you will be required to take a course of antibiotics after consulting with an ENT specialist.

Tension headaches also present with the above symptoms. The treatment lies with pain killers and reassurance. Exercises like yoga, counselling sessions can also help.

Since you have also mentioned about blurred vision, I would suggest you getting it checked, as visual disturbances can also result in headaches.

If I were your treating physician, I would start you on a drug named Amitriptyline in small doses. But since this drug causes addiction, you will not be able to get this drug from the pharmacy without a valid prescription. This drug is usually prescribed for long standing tension headaches.

I would suggest you to try this, after consulting with an ENT specialist for a proper prescription.
