What Causes Severe Headache And Tinnitus While On Tylenol?

Question: I keep getting these headaches everytime I lay down and rest my head on my arm. Each day or night, after about 10 minutes of resting my head on my hand/arm, I get a pretty bad headache right above my eyes and the top of my head. Tylenol barely helps. What can this be?

I can understand your concern. Your headache could any of below

- Stress headache - It is one of the most common type of headache. it can present like anything. Usually there is dull headache, band like sensation around forehead, heaviness on eyes, more pain on focusing eyes/looking at TV/laptop/mobile.
It can usually occur for long duration like complete day, less responsive to medication, mostly relieved after taking good sleep.

- Tension headache - It is similar to stress headache, only difference is it occurs mostly after chronic work fatigue.

- Brain tumor - The headache due to brain tumor has a tendency to increase on lying down due to increase blood flow in brain. It can present as dull headache, increased in morning/lying down, may cause projectile vomiting.

To rule it out, you must consult neurosurgeon and get MRI brain done to confirm.

- Brain vascular malformation - Brain vascular malformation involving eyes/frontal lobe like dural AV fistula, capillary hemangioma can present with increased headache. These lesions also cause pulsatile headache and ringing in ear.

These can be ruled out by MRI brain with MR angiography.

You should consult neurosurgeon as you are very young. Rule out any secondary cause of headache like tumor, vascular malformation.
Final treatment will depend upon confirmation of diagnosis. If still in doubt, do let me know.

It occurs every night, whether I had a good day at work or not. Almost every single time it happens, when I lay flat on my back and the back of my head on a pillow, the headache gets better within 10 minutes.

I forgot to mention that it could be due to hypertension. As it is occurring in occipital reason, please get your BP checked.

After going through your history, I can say that it is most probably stress headache (due to eye strain).

To confirm it further, you must consult neurosurgeon/neurologist. At least, get MRI Brain done to rule out the lesion.

The treatment of stress headache is

- Rest
- Avoid bright light
- Use sunglasses in day and use numbered glasses if you have vision problem
- Good sleep for 8-9 hours
- Avoid mental or physical stress
- Start exercise and Yoga after getting some relief
- You can take tablet Diclofenac with Paracetamol for few days and look for the response. If it is good, then you can take these medication intermittently after prescription.
