Question: What causes severe bladder pain after the first morning urination. I'm a 92 year old male.

I have gone through your query with diligence and would like you to know that I am here to help. There are a few causes for such a condition and I shall be more than happy to discuss them with you.

The most common symptom of a condition called interstitial cystitis (IC) is bladder pain after, during or before the morning urination. Let me also list the other common symptoms of IC for your reference, they are:

-Frequent urination
-Pain during sexual intercourse
-Pain or discomfort when the bladder is full or immediately after emptying
-Constant urge to urinate, but not much urine passed
-Sometimes a penile discharge
and a few other symptoms.

A urine examination, cystoscopy and sometimes even a biopsy may be required to confirm the diagnosis.

But if at all you do not have any of the other symptoms, then I would recommend an increased fluid intake. At times the problem could be simple dehydration, and so it would be wise to drink more fluids to help keep your urine diluted instead of concentrated.

Finally the last and other common cause for a presentation similar to yours is not urinating often enough as and when the urine accumulates in the bladder. If at all you think this is the cause, then please do try your best to urinate as and when you feel like it. Urine accumulated in the bladder can cause the stagnation of bacteria allowing it to breed and thrive there which first presents as a burning sensation in the bladder, and can later even result in infections.

So my suggestions would be the following:

-Try to empty your bladder as and when you feel the urge to urinate
-Try to drink at least 2-3 litres of water a day
-Get a urine culture and sensitivity test conducted to rule out/confirm IC
