What Causes Severe Abdominal Pain With Nausea When Diagnosed With Appendicitis?

Question: I have seen a doctor and I have apendecitis. He told me to take an anti-inflam tablet for pain and go home. The pain is worse at a score of about 9-10. I have about 4 pain contractions and hour with discomfort and nausea in between. Temperature is 36.6. Should I go to a hospital?

Dear,Based on the facts of your query,You need to shift to hospital and be Nil By Mouth.

-Hospitalize for Observation-
You need to be under observation of a ER Surgeon.
I would suggest investigations with USG Abdomen to rule out other causes and confirm the clinical findings with the CT Abdomen if need be.
Chances of Rupture of Appendix with old appendicular adhesions(causing 4 colicky contractions) is more in your case.

-Clinical Impression-
Acute on Chronic Appendicitis- as the Pains are of acute nature.The Frequency of 4 contractions indicates -adhesive bowels with chronic appendicitis.
Possibility of Kidney stone needs to be ruled out.
-Plan of Further treatment suggested-
To reduce on further impending complications, I would suggest Hospital Observation with -
Nil By Mouth-NBM
No painkillers for next 48 hrs, to reduce the chances of rupture of Appendicitis-as painkillers would damp down the real picture of Abdomen in your acute situation.
IV antibiotics if need be
IV fluids supplementary to maintain intake
Radiological(USG/CT Abdomen sos) and Blood /urine routine and microscopic tests -to assess the bacterial toxin effect on the body.
