Question: Hello Doctor,
Please help me out. I had sex with my girlfriend on friday evening, next day i had a severe itching problem, i was restless wholeday, today i pulled back my penis foreskin to see and i was literally very frightened when i saw that the my penis is full of red marks like scratches/cut, and it was dirty, white/grey particles were deposited all over, i tried to remove those but it were dry, so later i washed it with warm water, the dirt was removed maximum but red marks are all over. its itching and paining as well.

Could it be a STD?

Hi,
Welcome to health care magic,

It could be balanitis or balanoposthitis (infection of glans penis) due to friction or allergy but i will need to inspect the glans penis to come to a certain conclusion. STDs can easily be ruled out with blood tests.

So, kindly send me clinical picture of head of penis as there is feature given on the right side of query,so that i can guide you further accordingly.

The tip of my foreskin has a swelling now.
I tried to pull it back to clean and take a picture for your reference but I could not do so because of swelling. It was paining a lot.
Please suggest what should I do

Hi,
Thanks for the follow-up.

I can understand your pain and problem to take a picture.

It is likely because of balanitis or balanoposthoitis (infection of glans penis).

The important thing is to diagnose the underlying cause first by a clinical examination by a surgeon or dermatologist by doing inspection and palpation.

But following treatment would be helpful in the meantime.

1) Take Hot Sitz bath:

Sit for 15 to 20 minutes in warm water and clean off any clear or whitish secretion from the glans penis.

Avoid using soap to clean the glans as it may cause further irritation and pain.You can also use a moisturizing cream or ointment (an emollient) to clean instead of soap.

2. Local steroid with antiseptic cream/ointment application:

Clean off and wipe off the area dry following which you can apply local steroid ointment like betamethasone with neosporin (brand name: Betnovate-N)

The infection and swelling should start subsiding after 4 to 5 of ointment application.

Take over the counter pain killers like motrin (ibuprofen 400 mg)

3. Short course oral antibiotics like cephalosporins (cefadroxyl or cefixime) can also be taken but only after complete clinical examination by the doctor.

So, you should seek a help from local surgeon or dermatologist who will carefully inspect the glans penis with some routine blood works and tests for STDs and can above start treatment.

Good evening Doctor.

i took 2 pictures but how to send, i do not find any option to attach any file.
i am frightened, the swelling is too much..its paining very much and irritation and itching is also there. can not clean because i cant retract it (pull the foreskin back)

please suggest me a prescription.

Can not visit any doctor or have any blood tests over here as its a small town and you can understand my problem. i wont even be able to communicate with you also anymore as this is my 2nd and last followup :(

lastly please doctor, just tell me, is there any chance of STD?

Thanks for your reply again.

I totally understand your position and can empathize with the pain you are going through. You can ask help from our customer care team to send the images if you are not able to do so.

But without complete clinical examination I will not be able to make a clear diagnosis and it will be difficult for any doctor to do same only on the basis of history and descriptions,so thereby I need you to kindly understand the limitations for E-physician when it comes to dermatological problems.

More than any other field, in dermatology one needs to inspect with eyes what the lesions are, how they look, how the swelling, oozing, itching etc.

It was for that reason I had asked for a clinical photo in my first answer so that I can analyze the lesion and differentiate it from an infective lesion and give you a more direct and precise answer which would be satisfactory for both of us.

Hence I have to rely completely on your history and not the-best-of-descriptions can actually do justice, like having a single look at the lesion.

However,you can start the treatment which I have given you in my previous answer.

You can buy over the counter medications (pain killers,steroid with antiseptic ointment) directly from some nearby pharmacy and for prescription drugs (like antibiotics),you can see some local doctor or even GP who may also write it (if there is no surgeon or dermatologist in that small town).

Also,I am sorry to say that here on this Health Care Magic platform I will not be able to give you prescription as per universal E-patient policy.

As far as the STDs are concern,I will not be able comment without actual examination and relevant blood tests.
