Brief Answer:
It could be Varicose Veins and/or Osteoarthritis.

Detailed Answer:
Hi.
Thanks for your question.
She could be having Varicose Veins leading to the pain and there could be an element of Osteoarthritis in the knee due to her overweight status.
You should get an X-RAY of the affected knee done and get a Color Doppler of the venous system of that limb. These two investigations will tell us as to what should be done.
As for exercise cycling is definitely good, provided she can do it.
A photograph would be helpful but please take the picture when she is standing so that the veins in the leg become more prominent.
Take care and do let me know if there are queries on this issue

Brief Answer:
She will benefit from Compression Stockings and Tab Daflon

Detailed Answer:
Hi.
Good to hear from you.
I can't see any attached pictures.
The history you give is very suggestive of Varicose Veins and Chronic Venous insufficiency. Please get a Color Doppler of the Venous system of the lower limb done as I suggested earlier.
She would benefit from wearing Class II Compression stockings till above knee whenever she is standing or walking. Also you should start Tab Daflon 500 mg twice a day for three months. These measures will help her improve.
Advice her to sleep or lie down with her feet above her heart level. You could raise the leg end of the bed or out pillows below her feet.
Take care and do let me know the Doppler report.
