Brief Answer:
Most probably costo-chondritis.

Detailed Answer:
Hi.
Thanks for your query albeit short and specific.
You main complaint is pain under left ribs into left breast.
Also noted your history of Hiatal Hernia, and past history of multiple surgeries and medication history.

The commonest reason for the pain in the area you have mentioned is costochondritis.
Other reasons may be related to the heart, underlying lung or pleura.

If there is local tenderness (pain over the lower ribs) in the said area, most probable reason is costo-chondritis.
You may please test yourself, put a pressure by your own finger and let me know if the pain starts or increases.
Other causes can be ruled out by EKG, X-ray of the chest, clinical evaluation and actual examination by your Doctor.

I hope this answer helps you.
Please feel free to ask further relevant queries if you need to or if you feel that there is a gap of communication.
